ACCIDENT INVESTIGATION As part of the requirements of the Management of Health and Safety regulations a responsibility is placed on a company to carry out post accident investigations into the causes of any incident, and if there are actions that can be taken to prevent their re occurrence. To this end an investigation needs to look at all of the circumstances of the incident and find the root cause; equipment failure, human error, incorrect procedures, deliberate negligence, etc. You need to look at the previously carried out risk assessment to determine if this had been foreseeable or not, and that all reasonably practicable efforts had been made to prevent the occurrence. The risk assessment has to be reviewed in the light of the findings and if necessary changes should be made to procedures, equipment or training provided to prevent re occurrence. |
